Is Medicare (or single payer) more efficient?

It's not clear, if you include ALL the costs.  And even though I cannot find the link right now, NEJM seems to have found that once you adjust for the increased costs of Medicare Advantage (which runs on competitive pricing and premium support) it costs less then "base" Medicare.
